This is a program of assistance for older adults unable to remain
home during the day unsupervised. It's an opportunity to provide respite for
families caring for their loved ones and for the elderly experiencing social
isolation.
Annual costs are met through contributions
from individuals, City of Johnstown, City of Gloversville, Community Services
for the Elderly Grant from New York State, Long Term Home Health Care Program &
Fulton County Office for the Aging.
WHO IS ELIGIBLE?
Happy Days is open to Fulton County
residents 60 years and over who enjoy and benefit from supportive services
professionally supervised. Conditions resulting from strokes, blindness,
Alzheimer's Disease, as well as other dementias are cared for at the Happy Days
Adult Day Care Program.
PROGRAM OFFERS
All activities are designed to encourage
participation & motivation and to be fun.
Activities include: Creative arts & crafts, exercise, active games, music,
reality, morning nourishment, nutritious lunch, social casework, counseling,
referral (including lifetime planning).
ENROLLMENT
Referrals are made through Central
Assessment & Placement at 736-5718 or Happy Days Program at 762-4643. Staff at
Central Assessment will interview a prospective participant and determine if
this program would be appropriate for the individual's needs. If appropriate, a
trial visit will be set. Persons may attend weekly, daily or half days depending
on their needs.
The program is open from 9a-2:30p.
Monday-Friday.
COST
There is a suggested contribution by a
sliding fee scale. Persons are invited to help meet the cost of providing
services. No eligible person will be denied services for financial reasons.
